Description

As the Social Services Director, you will be responsible for overseeing social services within the facility. This role includes managing resident admissions, developing post-discharge plans, and ensuring compliance with all relevant social service regulations. You will also facilitate communication between residents, families, and staff, ensuring that all parties are informed and involved in care planning.

  • Complete necessary forms, reports, and evaluations related to social services.
  • Assist with resident admissions and develop comprehensive discharge plans.
  • Establish a quality assurance program for social services.
  • Manage and resolve abuse and grievance complaints, working closely with local agencies.
  • Maintain regular contact with families and hold individual conferences with residents.
  • Notify parties of impending evaluations and solicit their input during care plan conferences.
  • Counsel residents, family members, and staff on social service matters.

Qualifications

  • Bachelor's degree in Social Work (BSW) or a related field; LSW or LCSW licensure in good standing is required.
  • A minimum of one year of experience in a long-term care setting.
  • Strong communication skills and the ability to establish therapeutic relationships with residents.
  • Must not have an LPC or background in behavioral health, therapy, or counseling.
